Studi Kasus: Pilihan Modalitas Radiologis dalam Mendiagnosis Kasus Duplikasi Sistem Pieloureter

Duplikasi sistem pieloureter/duplicating collecting system merupakan salah satu kelainan kongenital pada traktus urinarius yang biasanya ditemukan secara insidental. Pemeriksaan foto polos abdomen-intravenous pyelografi (FPA-IVP) dapat memberikan gambaran anatomi yang jelas, Ultra Sono-Graphy (USG) dapat membantu memeriksa ginjal dan kantung kemih dengan baik, Computed Tomography (CT) Urografi dan Magnetic Resonance (MR) Urografi merupakan modalitas pilihan untuk mengevaluasi kelainan traktus urinarius karena dapat menggambarkan detail anatomi dengan baik. Penulisan kasus ini bertujuan untuk mengetahui gambaran dan pemanfaatan modalitas pemeriksaan yang tepat dalam mendeteksi kelainan kongenital terutama pada kasus traktus urinarius. Dilaporkan 2 pasien dengan pemeriksaan CT scan abdomen, dengan gambaran hidronefrosis dengan duplikasi pelvic calyceal system (PCS) dan bifid ureter pada kedua pasien, dimana insersi ureter distal pasien pertama pada vesika urinaria (VU) sedangkan pada pasien kedua insersi pada vagina. Gambaran khas pada duplicating collecting system meliputi fusi inkomplit dari upper dan lower moiety dengan variasi ureter seperti penyatuan duplikasi ureter sebelum masuk ke vesika urinaria, ureterokel maupun ektopik ureter yang menyebabkan terjadinya infeksi saluran kemih (ISK) berulang, refluks vesikoureter, hidronefrosis, hingga gangguan tumbuh kembang. Perawatan yang cepat dan tepat dapat mencegah komplikasi. Sebagai pilihan modalitas terbaik, CT/MR urografi mampu mendeteksi kelainan bawaan dan memberikan detail anatomi saluran kemih yang baik.Downloads
